Holmium Laser Enucleation of the Prostate (HoLEP) is a minimally invasive surgical procedure used to treat Benign Prostatic Hyperplasia (BPH), a noncancerous enlargement of the prostate that can obstruct urine flow. HoLEP offers a precise, long-term solution for patients experiencing moderate to severe urinary symptoms caused by prostate enlargement.
How HoLEP Works
Using a high-powered holmium laser, the surgeon removes the overgrown prostate tissue that is blocking the urethra. The tissue is then pushed into the bladder and removed using a device called a morcellator. This technique effectively opens the urinary channel, relieving pressure and improving flow.

Conditions Treated with HoLEP
- Benign Prostatic Hyperplasia (BPH)
- Urinary retention due to prostate obstruction
- Recurrent urinary tract infections caused by incomplete bladder emptying
- Bladder stones associated with BPH
- Inadequate response to BPH medications
Benefits of HoLEP
- Minimally Invasive: Performed through the urethra, with no external incisions.
- Effective for All Prostate Sizes: Especially beneficial for very large prostates.
- Low Risk of Bleeding: Suitable for patients on blood thinners.
- Shorter Hospital Stay: Most patients go home the same day or after an overnight stay.
- Durable Results: Low retreatment rates compared to other surgical BPH procedures.
- Improved Urinary Function: Most patients experience a significant improvement in flow rate and symptom relief.
What to Expect During and After the Procedure
HoLEP is typically performed under general or spinal anesthesia. The entire procedure usually takes about 90 minutes. A temporary catheter is placed to help drain urine during the initial recovery period and is generally removed within a day.
Patients may experience mild burning with urination, urgency, or minor blood in the urine for a few days. Most individuals return to light activity within a week and resume full activity, including sexual activity, after about 2–3 weeks. A small number of patients may experience mild temporary urine leakage that resolves within 3 months.
Is HoLEP Right for You?
HoLEP is ideal for men with significant BPH symptoms, especially those with very large prostates or those who wish to avoid long-term medication use. It also offers a safer option for patients who are not candidates for traditional surgery due to blood-thinning medications or other medical risks.
Talk to your urologist to determine whether HoLEP is an appropriate and effective option for your prostate health needs.
